right-hand side of your card to ensure you get the correct information.
If you will open your card up, there are "Disqualifications"
and "Exemptions" on
the back side. If any of the "Disqualifications" are applicable to
you, please circle the appropriate one and sign and date your card and mail back
to the County Clerk. If any of the "Exemptions" apply, and you wish to
claim the "Exemption", please circle the appropriate one and sign and
date your card and mail back to the County Clerk. There
is no need for you to call if you are claiming a "Disqualification" or
an "Exemption," just return your card to the County Clerk.
If you need to request a CHANGE for your Date of
Service, please contact the County Clerk's Office at (940) 627-1648.
