Please check the Date and Location of Service, which is located above your address on the right-hand side of your card to ensure you get the correct information.

Disqualifications & Exemptions

If you will open your card up, there are Disqualifications and Exemptions on the backside. If any of the Disqualifications are applicable to you, please circle the appropriate one and sign and date your card and mail back to the Court Clerk. If any of the Exemptions apply, and you wish to claim the Exemption, please circle the appropriate one and sign and date your card and mail back to the Court Clerk. There is no need for you to call if you are claiming a Disqualifications or an Exemptions just return your card to the Court Clerk.

Change Request

If you need to request a change for your Date of Service, please contact the appropriate Court Clerk identified in the return address on the left side of your card.
